Parchment, ii modern parchment flyleaves + 363 fols. + ii modern parchment flyleaves, 396 x 285 mm (300 x 191 mm), two columns, 48-50 lines, ruled in plummet, catchwords, running headers, one leaf lost before fols. 1, 13, 91, 153 and one after 363, two leaves lost before fol. 234. The text was written in iron-gall ink; small initials within the text were written with vermilion red and ultramarine blue.  

A physician, holding aloft a urine flask, is shown with five patients, including a woman with a heavily bandaged arm. Each of the ailments represented is discussed in the text.
